Reina Privacy Policy

Precognitive, Inc. (“Precognitive,” “Reina,” “we,” “us,” or “our”) provides Reina, a health information and care-coordination platform used by individuals and by participating healthcare providers, care organizations, pilot sites, and other organizations that purchase, sponsor, test, or provide access to the service.

This Privacy Policy explains how we collect, use, disclose, and protect information when you:

  • use the Reina mobile application;

  • use a Reina website or web-based service;

  • use Reina through a participating healthcare provider, care organization, pilot site, or customer;

  • connect health records or other data sources to Reina;

  • communicate with Reina through its AI-assisted features;

  • share information with a healthcare provider, caregiver, or other person; or

  • otherwise interact with Reina or Precognitive.

Please read this Privacy Policy carefully. By using Reina, you acknowledge the information practices described in this policy.

1. Information We Collect

The information we collect depends on how you access Reina, which organization provides or sponsors your access, and which features and data connections you choose to use.

Information You Provide Directly

We may collect information that you provide directly to us, including:

  • your name;

  • email address;

  • telephone number;

  • date of birth;

  • account credentials;

  • login and authentication information;

  • identity-verification information;

  • demographic information;

  • health concerns, symptoms, questions, and goals;

  • information entered into forms, surveys, or questionnaires;

  • information entered into Reina’s AI-assisted chat;

  • information about your healthcare providers, caregivers, family members, or care team;

  • documents, records, files, photographs, or other materials you upload;

  • consent forms, authorizations, sharing preferences, and communication preferences;

  • feedback, survey responses, and usability information; and

  • information included in customer-service or technical-support requests.

Health and Medical Information

With your permission, at the direction of a participating organization, or as otherwise permitted by applicable law, Reina may collect or process health-related information, including:

  • medical conditions and diagnoses;

  • health concerns and reported symptoms;

  • medications and medication history;

  • allergies and intolerances;

  • immunizations;

  • laboratory results;

  • imaging and diagnostic results;

  • procedures and treatment history;

  • appointments, encounters, and hospitalizations;

  • clinical notes and medical documents;

  • healthcare provider and care-team information;

  • referrals and care-coordination information;

  • insurance or claims information, when supported;

  • social and behavioral information relevant to care;

  • information from Apple Health or other health and wellness services;

  • information retrieved from patient portals;

  • information obtained through health-record retrieval services;

  • information obtained from healthcare organizations or health information exchanges; and

  • information obtained from other sources that you or a participating organization authorize.

The specific categories of health information available through Reina may vary depending on the participating organization, connected service, and features being used.

Information From Participating Organizations

If you use Reina through a healthcare provider, care organization, pilot site, employer, health plan, community organization, or other customer, that organization may provide us with information about you.

This may include:

  • identifying and contact information;

  • enrollment or eligibility information;

  • medical or care-coordination information;

  • appointment and referral information;

  • information needed to create or connect your account;

  • information needed to provide services through Reina; and

  • information concerning your participation in a pilot, program, or care service.

The organization may also instruct Precognitive to process information on its behalf, subject to the organization’s own privacy notice, consent forms, authorizations, agreements, and legal obligations.

Information From Connected Services

When you choose to connect an outside service, such as a patient portal, health-record provider, Apple Health account, health information exchange, or other health-data source, Reina may receive information according to the permissions you grant.

You may be able to limit or revoke a connection through:

  • Reina;

  • your mobile-device settings;

  • the connected service; or

  • the participating organization that manages the connection.

Revoking or disconnecting a source generally prevents future information from being transferred through that connection. It may not automatically delete information that was previously transferred to Reina or provided to a participating organization.

Technical and Usage Information

We may automatically collect limited technical and usage information needed to operate, secure, evaluate, and improve Reina, including:

  • device type;

  • operating system;

  • browser type;

  • application version;

  • IP address;

  • general network information;

  • login dates and authentication activity;

  • screens or features accessed;

  • button clicks and interactions;

  • dates and times of activity;

  • error reports;

  • crash information;

  • application performance data;

  • security logs;

  • audit logs; and

  • information about whether a feature was successfully completed.

We do not use personal health information for third-party targeted advertising.

Cookies, Analytics and Feedback

Our website and web-based services may use cookies or similar technologies for:

  • essential functionality;

  • authentication;

  • security;

  • remembering user preferences;

  • measuring service performance; and

  • limited analytics.

You may be able to control cookies through your browser settings. Disabling certain cookies may affect the functionality of the website or service.

Precognitive collects limited technical and product-usage information to maintain and improve Reina.

This may include:

  • application crashes and error reports;

  • device type, operating system, and application version;

  • the date and time an error occurred;

  • technical information needed to diagnose performance or reliability problems; and

  • optional feedback that you choose to provide, such as a thumbs-up or thumbs-down rating on a Reina chat response.

We use this information to:

  • identify and correct technical problems;

  • improve application reliability and performance;

  • evaluate whether Reina’s responses and features are helpful; and

  • improve the user experience.

Precognitive does not use advertising pixels, session-replay technology, or third-party targeted advertising within Reina.

Feedback ratings may be associated with the relevant Reina interaction so that we can understand what the rating refers to. Users are not required to submit feedback.

2. How We Use Information

We may use personal information to:

  • create, maintain, and administer Reina accounts;

  • authenticate users and verify identities;

  • provide Reina and its care-coordination features;

  • retrieve records from authorized sources;

  • connect and organize health information;

  • create health summaries, timelines, reports, and insights;

  • answer questions using AI-assisted features;

  • help individuals prepare for appointments or communicate with care teams;

  • support intake, referral, care-coordination, and follow-up workflows;

  • make information available to authorized personnel at participating organizations;

  • enable individuals to share information with healthcare providers, caregivers, and other people;

  • record and manage consent, authorization, and sharing preferences;

  • communicate about accounts, services, support, and security;

  • provide customer and technical support;

  • operate and administer pilots and customer programs;

  • evaluate whether Reina and its features are useful and functioning properly;

  • measure product use, performance, and outcomes;

  • test, maintain, troubleshoot, and improve the service;

  • prevent fraud, unauthorized access, and misuse;

  • investigate security incidents;

  • comply with legal, regulatory, contractual, and healthcare obligations;

  • enforce our agreements and policies; and

  • protect Precognitive, participating organizations, users, and others.

We will not use personal information for a materially different purpose without providing any notice or obtaining any consent required by applicable law.

3. AI-Assisted Features

Reina uses artificial intelligence to help users and participating organizations:

  • organize health information;

  • summarize records;

  • identify potentially relevant information;

  • generate timelines or overviews;

  • answer health-information questions;

  • prepare for appointments;

  • support intake and care coordination; and

  • create other informational outputs.

AI-generated information may be incomplete, inaccurate, outdated, or inappropriate for a particular person or situation.

Reina does not replace a physician or other qualified healthcare professional. Reina should not be used as the sole basis for diagnosing a medical condition, selecting treatment, changing medication, or making emergency decisions.

If you believe you are experiencing a medical emergency, call 911 or seek immediate medical assistance.

Information may be processed by service providers that supply artificial-intelligence, hosting, database, or cloud-computing infrastructure to Precognitive. These service providers may process information only as necessary to provide contracted services to us and are subject to applicable confidentiality, privacy, and security requirements.

Precognitive may use your health data to train our AI systems, you can opt out of this. If you revoke consent, you can choose to delete all data or allow precognitive to retain the de-identified data for the purposes of training.

Precognitive does not permit third-party AI providers to use identifiable personal health information to train their general-purpose AI models.

4. How We Disclose Information

We may disclose personal information in the circumstances described below.

Participating Organizations and Partners

Precognitive works with healthcare providers, care organizations, pilot sites, health plans, community organizations, and other organizations that purchase, sponsor, test, or provide access to Reina.

When you use Reina through one of these participating organizations, we may disclose your personal information, including health-related information, to that organization as necessary to:

  • provide Reina and its care-coordination features;

  • connect and organize your health information;

  • provide authorized personnel with access to your Reina information;

  • create summaries, timelines, insights, reports, or other outputs;

  • support intake, assessment, referral, care coordination, treatment, and follow-up;

  • communicate health concerns or requests;

  • allow the organization to provide services to you;

  • administer and evaluate a pilot or service relationship;

  • measure use, performance, outcomes, and program effectiveness;

  • provide support and maintain the service;

  • satisfy our agreements with the organization; and

  • comply with applicable legal requirements.

Depending on the program, an authorized person at the participating organization may be able to view information in your Reina account without you manually sending each item.

The participating organization determines which members of its workforce are authorized to access information made available through Reina. Its own privacy policies, notices, consent forms, authorizations, and legal obligations may apply to information it receives.

We will provide any notice and obtain any consent or authorization required by applicable law or by the applicable pilot or service program.

Participating organizations are not permitted to use information received through Reina for unrelated advertising or other purposes inconsistent with the applicable service, agreement, consent, or authorization, except where separately disclosed and legally permitted.

At Your Direction

We may disclose information when you direct or authorize us to:

  • send a health summary, report, link, PDF, or record to a healthcare provider;

  • connect your Reina account with a participating organization;

  • share information with a caregiver, family member, or other representative;

  • connect an outside health-data source;

  • participate in a pilot, care program, or research activity; or

  • otherwise disclose information to a person or organization you select.

You should carefully review the recipient before sharing information.

After information is delivered to another person or organization, its further use may be governed by the recipient’s privacy practices, professional obligations, or applicable law rather than solely by this Privacy Policy.

Service Providers

We may disclose information to vendors and contractors that help us operate Reina, including providers of:

  • cloud hosting;

  • secure data storage;

  • database infrastructure;

  • authentication and account management;

  • health-record retrieval;

  • health-data interoperability;

  • artificial-intelligence processing;

  • application monitoring;

  • error and crash reporting;

  • communications;

  • customer support;

  • security;

  • compliance;

  • legal services; and

  • auditing.

We require service providers to process information only for authorized purposes and to use appropriate privacy and security safeguards.

Legal, Regulatory, and Safety Reasons

We may disclose information when we reasonably believe disclosure is necessary to:

  • comply with applicable law or regulation;

  • comply with a court order, subpoena, legal process, or governmental request;

  • comply with healthcare reporting or recordkeeping obligations;

  • investigate suspected fraud, abuse, or misuse;

  • investigate or respond to a security incident;

  • enforce our agreements or policies;

  • establish, exercise, or defend legal claims;

  • protect the rights or property of Precognitive or a participating organization; or

  • protect the safety of users or other people.

Business Transactions

Personal information may be transferred or disclosed as part of:

  • a merger;

  • acquisition;

  • financing;

  • restructuring;

  • bankruptcy;

  • sale of company assets;

  • due-diligence review; or

  • similar business transaction.

Any recipient of the information would be required to handle it in accordance with applicable law and any binding privacy or contractual obligations.

Where required, we will provide notice or obtain consent before information becomes subject to materially different privacy practices.

5. Selling Personal Information and Advertising

Precognitive does not sell personal health information for money.

Precognitive does not use personal health information for third-party targeted advertising.

Precognitive shares personal information with participating organizations because those organizations provide, sponsor, pilot, purchase, or use Reina to deliver services. This operational disclosure is different from selling information to unrelated data brokers or advertisers.

6. HIPAA and Healthcare Organizations

The Health Insurance Portability and Accountability Act, commonly known as HIPAA, applies only in certain circumstances.

When Precognitive provides Reina to a healthcare provider, health plan, or other organization regulated by HIPAA, Precognitive may act as that organization’s business associate. In those circumstances, Precognitive processes protected health information according to:

  • HIPAA;

  • an applicable business associate agreement;

  • the participating organization’s instructions; and

  • other applicable healthcare privacy and security requirements.

In other circumstances, information that an individual independently enters into or connects with a consumer-directed Reina account may not be protected by HIPAA.

Even when HIPAA does not apply, other federal and state privacy, security, health-data, consumer-protection, and breach-notification laws may apply.

A participating organization may provide you with its own Notice of Privacy Practices or other privacy notice. That notice describes how the organization uses and discloses information under its control.

Where an organization-specific privacy notice, authorization, or agreement applies to particular information, that document may govern in addition to this Privacy Policy.

7. Consent and Authorization

Depending on the service or participating organization, we may ask you to provide consent or authorization before:

  • retrieving health records;

  • connecting a patient portal or data source;

  • accessing Apple Health information;

  • disclosing information to a participating organization;

  • sharing information with a caregiver or other person;

  • using information for a pilot or research activity; or

  • enabling an optional feature.

You may be able to withdraw certain consents or authorizations through Reina or by contacting us at admin@precog.works.

Withdrawal generally applies to future collection, use, or disclosure. It does not necessarily require Precognitive or a participating organization to reverse actions already taken in reliance on a valid consent or authorization.

Withdrawal may also limit or prevent your continued use of certain Reina features or participating-organization services.

8. Your Choices and Controls

Depending on the features available, the organization providing your access, and applicable law, you may be able to:

  • access your account information;

  • update or correct account information;

  • review connected data sources;

  • disconnect a data source;

  • withdraw an optional consent;

  • change sharing preferences;

  • request a copy of your information;

  • download certain information;

  • request correction of inaccurate information;

  • request deletion of your account;

  • request deletion of certain information;

  • opt out of nonessential communications; and

  • submit a privacy request.

We may need to verify your identity before fulfilling a request.

If information is controlled by a participating healthcare provider or other organization, we may direct your request to that organization or work with the organization to respond.

Disconnecting a data source or withdrawing consent may not delete information that was previously:

  • collected by Reina;

  • incorporated into an existing summary or record;

  • disclosed to a participating organization;

  • included in legally required audit or security logs; or

  • retained under a healthcare, legal, contractual, or regulatory obligation.

9. State Privacy Rights

Residents of certain U.S. states may have additional rights regarding personal information or consumer health data.

Depending on where you live, the nature of the information, and whether the applicable law covers Precognitive, these rights may include the right to:

  • confirm whether we process your information;

  • know what categories of information we collect;

  • access information we maintain about you;

  • receive a portable copy of certain information;

  • correct inaccurate information;

  • delete certain information;

  • withdraw consent;

  • obtain information about certain third parties that received your data;

  • opt out of certain sales, targeted advertising, or profiling;

  • restrict certain uses or disclosures of consumer health data; and

  • appeal a decision concerning a privacy request.

Precognitive will not unlawfully discriminate against you for exercising an applicable privacy right.

If your request relates to information controlled by a participating organization, we may refer the request to that organization.

10. Data Retention

We retain personal information only for as long as reasonably necessary for the purposes described in this Privacy Policy, including:

  • providing Reina;

  • maintaining user accounts;

  • providing services to participating organizations;

  • supporting healthcare and care-coordination workflows;

  • maintaining security and audit records;

  • complying with legal, contractual, healthcare, and regulatory obligations;

  • resolving disputes;

  • responding to privacy requests; and

  • enforcing our agreements.

The applicable retention period may depend on:

  • the category and sensitivity of the information;

  • whether your account remains active;

  • the participating organization through which you use Reina;

  • the source from which the information was obtained;

  • your consent and sharing choices;

  • customer and pilot agreements;

  • healthcare recordkeeping requirements;

  • applicable law; and

  • security and fraud-prevention needs.

Some information may be retained after an account-deletion request when necessary to:

  • comply with law;

  • preserve legally required healthcare records;

  • maintain security or audit records;

  • document consent or authorization;

  • prevent fraud or misuse;

  • resolve disputes; or

  • enforce our agreements.

Where appropriate, information may be deleted, de-identified, or aggregated when it is no longer needed.

11. De-Identified and Aggregated Information

We may create information that has been de-identified, aggregated, or otherwise modified so that it does not reasonably identify an individual.

We may use properly de-identified or aggregated information to:

  • evaluate service performance;

  • understand how Reina is used;

  • improve product functionality;

  • conduct internal research and analysis;

  • measure pilot or program outcomes;

  • develop new features; and

  • prepare reports for participating organizations.

Where required by applicable law, we will maintain de-identified information in de-identified form and will not attempt to re-identify it.

12. Security

Precognitive uses administrative, technical, and physical safeguards designed to protect information from:

  • unauthorized access;

  • loss;

  • misuse;

  • alteration;

  • destruction; and

  • unauthorized disclosure.

These safeguards may include:

  • encryption during transmission and storage;

  • access controls;

  • authentication;

  • role-based permissions;

  • security and audit logging;

  • employee and contractor confidentiality requirements;

  • vendor-security reviews;

  • secure software-development practices;

  • incident-response procedures;

  • backups; and

  • workforce privacy and security training.

No system, storage method, or transmission method is completely secure. Therefore, we cannot guarantee absolute security.

You are responsible for:

  • protecting your login credentials;

  • maintaining the security of your device;

  • using appropriate device passcodes and security settings;

  • not sharing authentication codes; and

  • notifying us if you believe your account or information has been compromised.

13. Data Breach Notifications

If Precognitive determines that personal information has been involved in a breach or security incident requiring notification, we will provide notifications as required by applicable law.

Depending on the circumstances, notifications may be provided to:

  • affected individuals;

  • participating organizations;

  • healthcare organizations;

  • government agencies;

  • regulators; or

  • other legally required recipients.

14. Children’s Privacy

Reina is not intended for use by children under the age of 13 unless the service is provided through:

  • an authorized parent or legal guardian;

  • a healthcare provider;

  • a participating organization; or

  • another program with appropriate consent, authorization, and safeguards.

We do not knowingly collect personal information directly from a child under 13 without appropriate authorization.

15. Third-Party Services and Links

Reina may connect to or contain links to websites, applications, patient portals, healthcare services, and other services operated by third parties.

Third-party services have their own privacy policies, terms, and security practices. This Privacy Policy does not govern information that a third party independently collects or controls.

We are not responsible for the privacy or security practices of third-party services that you independently access.

When you authorize Reina to connect with a third-party service, information exchanged through that connection will be handled as described in this Privacy Policy and in the third party’s applicable privacy notice.

16. Communications

We may send you service-related communications concerning:

  • your account;

  • authentication;

  • data connections;

  • consent or authorization;

  • information-sharing activity;

  • security;

  • changes to Reina;

  • participating-organization services;

  • support requests; and

  • changes to this Privacy Policy.

You may not be able to opt out of communications that are necessary to operate or secure your account.

We will obtain any consent required by law before sending promotional or marketing communications. You may opt out of promotional emails by using the unsubscribe instructions in the message or by contacting admin@precog.works.

17. International Use

Reina is currently intended primarily for use in the United States.

If you access Reina from outside the United States, your information may be transferred to, stored in, or processed in the United States, where privacy laws may differ from those in your location.

18. Changes to This Privacy Policy

We may update this Privacy Policy to reflect changes to:

  • Reina;

  • our information practices;

  • participating-organization services;

  • legal or regulatory requirements; or

  • our vendors and technology.

When we update the policy, we will revise the “Last Updated” date at the top.

If a change materially affects how we collect, use, or disclose sensitive information, we will provide additional notice and obtain consent when required by applicable law.

We encourage you to review this Privacy Policy periodically.
